To Our Loyal Clients and Beloved Pets:

It is with our deepest regret that we are forced to announce that we are closing the Animal Hospital. The crushing economic recession has made it impossible for us to continue business and has forced the hospital into bankruptcy. With great sorrow, our doors are now closed.

We will miss the relationships forged with our clients and all opportunity to provide on-going medical care and health-wellness services. It has been our pleasure to serve you and your wonderful pets. Each and every one of you will be missed. As fellow animal lovers, your pets have a special place in our hearts.

To those among our professional community, We at the Animal Hospital have greatly enjoyed the professional relationship we have had with you, and we deeply appreciate the wonderful support and services many have given us during the last 3 years in which we operated the hospital. We maintain great pride in the fabulous team of support staff we had gathered at the very end and deeply regret losing them by our closing. Every one of them has left with our highest recommendation, and we hope you will consider them when you have need of great support staff and wonderful people.

To Obtain a Copy of Your Pet's Medical Records:

It is our hope that clients seek on-going care at the San Francisco SPCA. We recognize the immensely important role this organization plays throughout the San Francisco Community. Furthermore, we will always appreciate the relationship we have forged with the SPCA during our time of hospital ownership and know this organization is invaluable to every emergency, specialty and general practice hospital.

Among our final thoughts of concluding business—having toured their beautiful new hospital and met their terrific team—we are confident and happy to recommend this hospital and its doctors and support staff. The San Francisco SPCA has graciously offered to host our client records as well as welcome our prior patients for on-going medical, wellness and preventative care. Please contact the SFSPCA at 415-554-3030 and let them know you were a previous client at our hospital.

All written requests for your pet's medical records (within last 3 years) will be made available as quickly as possible. We value your patience as urgent medical requests may be addressed first. For the security of your pet(s) medical records and the safety of confidential information, please provide the name of owner(s) and the yearly age of your pet. Please respond in the following way:

Email request at diamondheightspets@gmail.com. We will reply with your pet's records attached in PDF format as soon as we can. (Email enables us to provide records efficiently and freely)

Otherwise, clients can submit a written request to our post office box. We will mail a CD containing records of your requested pets in electronic PDF format. (Please note: since our offices are closed for all business activity, there will be a delay in processing mailed requests, and we will need to collect a $3 fee to cover the costs of compact disk reproduction and mailing.)
